The Montana Department of Public Health and Human Services will collaborate with rural providers, hospitals, community organizations, and other health care stakeholders to deliver five core initiatives tailored to Montana’s rural health care challenges.

$20M in funding for implementation-ready projects that expand access to specialized care, reduce hospitalizations, and strengthen rural healthcare partnerships. Awards up to $4M, no match required.

• Ohio Rural Health Transformation Program

$14.6M in funding to improve chronic disease outcomes in rural Ohio, with support for coordinated care teams, remote monitoring, and digital health tools. Awards up to $200K per clinic.
